Jay's Support Services

For support coordinators, behaviour support practitioners, allied health and justice teams

Send us the referral you’re having trouble placing.

Complex behaviours, a forensic history, a transition back into the community. This is the work Jay’s Support Services was built for, and our team was doing it long before the NDIS existed.

What you get

Seven things you can hold us to.

A straight answer on suitability

We’ll tell you on the call what we can take and what we can’t. You get the read before you build a plan around it, not three weeks in.

You’re talking to the people who do the work

There’s no intake queue and no account manager. The person who takes your call does the assessment, and they’ll be at the review too.

We turn up to the care team

Fortnightly visits where someone’s in custody, transition planning alongside corrections and forensic mental health, and the same faces at every meeting.

Workers are briefed before they start

Anyone coming into a complicated situation gets the full background first, in person, including the parts that are hard to hear. Nobody arrives cold.

We don’t hand people back

The people we support are usually the ones a previous provider gave up on. Nobody has left Jay’s Support Services since it started.

We take the transition, not just the settled part

Release, discharge, a move out of crisis accommodation. The stretch where support normally changes hands is the stretch we are built for.

We work with the whole support network

Family, carers and the professionals already involved, where the participant wants them there and consents to it. You are not handing the person to a provider who works in isolation.

When to call us

The referrals other providers return.

  • A forensic history, or someone currently in custody preparing for release
  • Behaviours of concern that have cost the person their last placement
  • Substance use running alongside a disability, where both need holding at once
  • Obligations that cannot slip: reporting appointments, court dates, treatment, and other people who need to know how things are going
  • Someone who has been handed back more than once and is losing faith in providers
  • A plan that needs a change of circumstances submitted and nobody driving it

If it is none of these, call anyway. We also support people whose needs are entirely physical, and the honest answer on fit takes about five minutes.

Registration and safeguarding

The things you check before you call.

Certificates of currency, our policies and a capability statement are available on request. Ask and we will send them.

  • A registered NDIS provider

Jay’s Support Services Pty Ltd, ABN 33 662 241 723. Registered against the NDIS Practice Standards and bound by the NDIS Code of Conduct.

  • Every participant is met before a worker is placed

The match is made deliberately, on the person’s full background, and it is changed early when it is wrong.

  • Consent leads every conversation

Nothing is shared with family, health services or justice teams without the participant’s consent. Where they want people in the room, we bring them in.

  • We work under the plan, not around it

Where there is a behaviour support plan we implement it alongside the practitioner who wrote it, and we stay in contact with them.

Make a referral

Tell us the bit that makes it hard.

You do not need a full referral pack to start. A paragraph on the situation and what has already been tried is enough for us to tell you whether it is a fit.

Areas covered
