What we do

Support built around one person at a time.

Jay’s Support Services is a registered NDIS provider delivering core supports and support coordination across Victoria. Small caseload, deliberately. Our team knows every person on it.

In-home support

For some people the hard part is being on their own. One of the people we support needs somebody there most of the day, every day, because isolation is the thing that undoes him. So that is what the roster says.

  • Daily living, personal care and the routine that holds a week together
  • Company, and somebody to talk to, which for some people is the actual support need
  • Being there for the appointment, the shop run and the thing that keeps getting put off

Community participation

Getting out is not a nice-to-have bolted onto the plan. It is most of what changes how a week feels. The gym on a Tuesday, the boxing bag, a match on the weekend, a coffee somewhere ordinary.

  • Gym and sport, including the ones people actually want to do rather than the ones that look good in a plan
  • Appointments, and staying in the room for them
  • The slow rebuilding of a life that has somewhere to be

Support coordination

For most of the people we support, we coordinate as well. That means the care team meeting actually happens, and somebody who sees the person every week is in the room when decisions get made.

  • The OT, the behaviour support practitioner, the lawyer and the family in one conversation, with the participant deciding who is in it
  • Chasing the things that stall: plan reviews, change of circumstances, a BSP who will take the referral
  • Translating what a plan says into what a week looks like

Staying through the transition

Custody back into the community. Crisis accommodation to a front door of your own. Hospital back home. These are the stretches where support usually changes hands, and where changing hands does the most damage.

  • Fortnightly visits while somebody is still inside, so the relationship already exists on release day
  • Transition planning alongside corrections and forensic mental health
  • Finding accommodation, and then staying on afterwards rather than closing the file

Out in it

Most of the work happens outside a house.

The gym, the boxing bag, a powerchair football match. It looks like an ordinary week because that is exactly the point. These are our own clips, taken on shift by the people who were there.

Who we support

Adults and young adults whose situations are usually described as too complicated.

People coming back into the community from custody. People whose behaviour has cost them their last two providers. People managing substance use alongside everything else. People who have been handed back more than once.

Wherever the participant wants it, we work alongside their family, their carers and the professionals already involved. Understanding the whole person, and not just the referral, is what makes the support hold.

We also support people whose needs are entirely physical, and people whose situation only became complicated after an accident. There is no screening test. We take the call and give you a straight answer.

Where we work

Based in Werribee, working across Victoria.

Melbourne metro and Geelong every week, and Geelong is three days of it rather than an occasional visit. We have the capacity to support participants elsewhere in the state where it is the right fit. Call and we will tell you honestly whether we can cover it.
